Description: The Home and Community-Based Waiver program provides Medicaid coverage to eligible persons who are aged or disabled, who would otherwise require nursing facility level of care. Services include but are not limited to the following: assessment and reassessment, case management, homemaker services, personal care services, respite care, minor home adaptations, attendant care, and adult day health care.
