Description: The Acquired Brain Injury Long Term Care Waiver program provides an alternative to institutional care for individuals who have reached a plateau in their rehabilitation level and require maintenance services to avoid institutionalization and to live safely in the community. Services include case management, community living supports, respite care, adult day health care, adult day training, environmental modifications, behavior programming, counseling, group counseling, specialized medical equipment and supplies, supported employment, occupational therapy, speech therapy, nursing supports, family training, physical therapy, assessment and reassessment, and supervised residential care.
